Permission to use likeness on Teen-Beat 119.

Ivo Watts-Russell from 4AD had remembered photographs of Cath Carroll taken by Robert Mapplethorpe from the late 1980s or early 1990s and thought one of them would be perfect for UNREST's Perfect Teeth album. The Robert Mapplethorpe Foundation was contacted and licensing was obtained for one of the photographs of Cath Carroll.

Ivo then contacted Cath Carroll, the subject of one of the songs on the album, and received her permission to appear on the cover of the album.
